The Margin Math Every Board Is Already Doing

Somewhere in your organization, a board member is doing this math on a napkin, whether you've handed them the numbers or not.

A recent national survey of 100 hospital and health system CFOs found that 72% report operating margins of 2% or less. That's not a crisis at any one hospital — it's the baseline condition of the industry right now. When I was in the chair, that thin a margin meant every capital request, every staffing decision, and every new service line got read through one lens: does this move the margin, or does it just feel productive?

Boards know this instinctively, even when they can't cite the survey. That's why board meetings increasingly feel less like strategy sessions and more like margin autopsies. If you're a CEO right now, the uncomfortable truth is that your board is already running a version of this math on your organization — and the only question is whether you get to frame the conversation or react to theirs.

The same survey found the top drivers of financial pressure aren't mysterious: declining reimbursement and payer mix shifts (45%), reduced government funding and regulatory risk (42%), rising labor costs (40%), and underutilized capacity or inefficient patient throughput (39%). None of those are one-time problems you fix with a single initiative. They're structural, which means the response has to be structural too — not a single cost-cutting sprint before the next board meeting.

What I'd tell a CEO in this position: don't wait for the board to ask for the margin plan. Bring them the math first, in plain language, with a short list of levers you're already pulling and an honest read on which ones will move fastest. Boards forgive thin margins. They don't forgive being surprised by them.
